Abstract

Continuous glucose monitoring (CGM) use is rapidly growing worldwide during past decade. Robust evidence from high-income countries has made CGM as the standard of care especially in individuals with type 1 diabetes (T1DM). Nonetheless, scarce evidence from resource-limited countries underscores the formulation of country-specific expert consensus recommendations. This expert consensus aims to provide country-specific recommendations for the integration of CGM into the outpatient diabetes care in Iran. An expert task force conducted a comprehensive literature review and drafted the initial recommendations. Feedback from 50 nationally recognized clinicians and expert endocrinologists implemented into the final draft, which was endorsed by the Iran ministry of health and medical education. This consensus recommendations are categorized into affordable and unaffordable settings. The frequency of CGM use is tailored in three modalities including continuous, short-term continuous and intermittent use, based on the individual risk factors such as age, type of diabetes, level of glycemic control, hypoglycemia risk and pregnancy status. This expert consensus fills the gap between international CGM guidelines and Iran's healthcare realities, reflecting both clinical priorities and economic barriers, hence acting as a guide for both clinicians and policy makers to integrate CGM use in diabetes care. © 2025 Elsevier B.V., All rights reserved.
