Comparison of Endovascular Aneurysm Repair (Evar) With Open Surgical Repair in Elective Patients With Abdominal Aortic Aneurysm Performed by Vascular Surgeons: One-Year Outcome Publisher

Abstract

Background: Open surgical repair and endovascular aneurysm repair (EVAR) are considered as main options for surgical treatment of abdominal aortic aneurysm (AAA). Typically, endovascular repair is performed in patients with a high risk for surgery. The aim of this study was to compare the results and complications of these surgeries, both being done by vascular surgeons in patients with abdominal aortic aneurysm. Methods: The current prospective study was conducted on AAA patients that had elective surgery in Sina hospital (Tehran, Iran) from September 2011 to September 2013. The data was collected from the patient documents and one year of follow-up was done by calling patients or their families. Results: Thirty-six patients were included in our study (open-surgery group: 14 patients (38.9%) and EVAR group: 22 patients). The two groups were similar considering age, sex, comorbidities, cardiopulmonary and pulmonary drug usage, as well as lab tests. In the open surgery group, three cases (inside the hospital) and in the EVAR group, six cases (two cases inside and four cases outside the hospital) died within the first year. There was not any significant differences between two groups regarding the frequency, place and cause of death. However, the length of hospitalization after the procedure, in the intensive care unit and ward was significantly lower in the endovascular group compared to the open-surgery group. Conclusion: Our study suggests that the overall mortality rate is almost the same in EVAR and open surgery groups. However, it should be noted that the length of stay in the hospital (considering ward and ICU) is significantly shorter in patients with EVAR. Therefore, it can be recommended as a better option in patients with appropriate pre-operative conditions. © 2020, Advanced Scientific Research. All rights reserved.
