Abstract

Background: Early clinical exposure (ECE) is increasingly recognized as an effective educational strategy to bridge the gap between preclinical learning and clinical practice in dental education. This systematic review aimed to synthesize the global evidence on the impact of ECE on undergraduate dental students. Methods: A systematic search was conducted in PubMed, Scopus, and Web of Science from inception to October 2025. Studies evaluating ECE interventions among dental students at any stage of preclinical education and reporting educational or professional outcomes were included. Data were extracted and synthesized narratively, and study quality was appraised using the Joanna Briggs Institute (JBI) tools. Findings: Twelve studies met the inclusion criteria, including randomized controlled trials, quasi-experimental studies, cross-sectional surveys, and qualitative research. ECE interventions varied from direct patient contact and clinical rotations to structured preclinical modules, peer-assisted learning, and video-based demonstrations. Most studies reported significant improvements in students’ knowledge, psychomotor skills, confidence, communication abilities, and motivation. Students also reported enhanced readiness for clinical practice and stronger professional identity. Common challenges included faculty shortages, curriculum integration difficulties, and limited use of validated assessment tools. Conclusion: ECE has a consistently positive impact onmultiple domains of dental education and supports a smoother transition from classroom learning to clinical practice. Incorporating structured and patient-centered ECE components into dental curricula is strongly recommended to improve educational outcomes and professional development. © 2026, Isfahan University of Medical Sciences(IUMS).
