Abstract

Vitamins are linked to a reduction in clinical manifestations and risk of Amyotrophic lateral sclerosis (ALS). This narrative review aimed to explore the role of vitamin supplementation and serum vitamin levels in relation to functional status, respiratory function, and survival in pwALS. All English observational and interventional human papers on the impact of vitamins encompassing water- and fat-soluble vitamins were gathered from valid scientific databases. Serum concentrations of vitamins are considered to be associated with ALS progression through oxidative stress pathways and neuroinflammation. Additionally, vitamin supplementation may have a key role in modifying ALS-related clinical manifestations. Existing evidence has indicated an inverse link between the supplementation of vitamins E and D and clinical symptoms, as well as the risk in people with ALS (pwALS). Contradictory results have been reported about the role of B vitamins in pwALS. This narrative review emphasizes the significance of both serum concentrations and supplementation of vitamins in pwALS. Further studies are warranted. © 2026 The Author(s).
