Abstract

Introduction: This study aimed to investigate and compare the outcomes of the first and second ear stapedotomy in patients with bilateral otosclerosis at Tehran University of Medical Sciences during 2022-2023. Methods: This retrospective cohort study included 31 patients with otosclerosis referred to the ear, nose, and throat clinic of Imam Khomeini and Amir Alam hospitals in Tehran who underwent binaural surgery. Data were collected using a checklist that included demographic information and complications such as dizziness, tinnitus, and pain. The data were analyzed using SPSS software. Results: The study included participants with a mean age of 42.1 ± 11.6 years. Postoperative Air-Bone Gap (ABG) at 500, 1000, and 2000 Hz was lower in the first-ear surgery group compared to the second, indicating better hearing outcomes, with significant differences at 500 and 1000 Hz. Bone conduction thresholds improved significantly at 2000 Hz after surgery, with no difference between ears. The level of patient satisfaction with surgery had no significant relationship with the complications of tinnitus-pain-vertigo surgery in any of the surgeries. The level of satisfaction in the first surgery had a significant relationship with GAP-Air Bone at frequencies of 500 and 1000, and this relationship was seen with Air Bone GAP at 2000 frequencies in the second surgery. There was no significant difference between the level of satisfact ion of the first and second surgery. Among the complications, the difference in tinnitus complications in the first surgery was significantly higher than in the second surgery. Conclusion: No significant difference was found between the two ear surgeries; only the hearing result (post op GAP) was the only factor affecting patient satisfaction. © 2025, Shahroud University of Medical Sciences.
