Situation Analysis and Operational Strategies for Improving the Mental-Behavioral Health of the Elderly and Caregivers Publisher

Abstract

With the rapid increase in the elderly population in Iran, the home care system has faced significant challenges. Home caregivers, who are mainly family members, face significant psychological, economic, and social pressures that lead to burnout, depression, and even the development of chronic diseases in them. On the other hand, the elderly face multiple challenges, including chronic physical illnesses, mental disorders, social isolation, and in some cases, abuse. Thematic analysis in this document indicates that improving the situation of the elderly and their caregivers requires urgent and multi-level policy interventions. In the short term, training in caregiving skills and strengthening mental health services in health centers can have the greatest impact. Implementing these strategies is relatively simple, and they are also cost-effective. In the medium term, developing online digital support systems could improve access to counseling services, although the technological limitations of older people should be taken into account. Financial support for caregivers, although desirable, was introduced as a lower priority due to the implementation challenges and the need for a strong monitoring system. The document emphasizes that improving the current situation while simultaneously considering the needs of older people and caregivers is possible. An integrated approach that both enhances caregiving skills and strengthens psychosocial support services can prevent caregiver burnout and improve the quality of life of older people. Phased implementation of these strategies is the most practical path to creating sustainable change in this area, given resource limitations and existing infrastructure. Health policymakers should note that investing in this sector is a humanitarian necessity and a preventive measure to reduce future healthcare system costs. © 2025 Elsevier B.V., All rights reserved.
